Include the names of your loved ones in the Pre-Yom Kippur Yizkor program

Complete this form to submit names of your beloved members of your family or friends to be remembered during Yizkor Services at Chabad during the Jewish Calendar Year of 5784. Yizkor is recited four times: Yom Kippur, Shmini Atzeret, Last day of Pesach, and on the Second Day of Shavuot.

The names you submit will be included in a beautiful Yizkor booklet to be printed for our Chabad Shul and in our prayers.

All submissions must be received before Thursday September 21, 2023 12:00 Noon, as the Yizkor Booklet must be completed and sent to print.
